Success Story: Sandra

Sandra Welch to Chicopee Gardens following a hospitalization due to a bad fall. Lacked stability in her legs and having trouble walking – Sandra needed help.

Needing help is something Sandra knew quite well. You see, Sandra had previously served as a nurse at this very facility and has seen the challenges and successes of rehabilitation first hand. Through her experience, she also understood the importance of having a dedicated and professional therapy team.

Working with Sandra, our physical and occupational therapy teams developed a comprehensive rehabilitation plan with strategic objectives – our goal was to help Sandra gain strength and stability in her legs and return home.

We are proud to say that Sandra has met her therapy goals and has returned home!

Public Announcement: On April 1, 2019, the Department of Public Health, Division of Health Care Facility Licensure and Certification (the “Department”) received notification of a proposed change in ownership of the Chicopee Gardens Rehabilitation and Care Center, 44 New Lombard Road, Chicopee, MA 01020. Chicopee Rehabilitation and Nursing Center, LLC, 44 New Lombard Road, Chicopee, MA 01020, if deemed suitable by the Department, intends to purchase the nursing home from the current operator, Chicopee Gardens Rehabilitation and Care Center Operations LLC. There will be no changes in the bed capacity or the services provided by Chicopee Gardens Rehabilitation and Care Center. A public hearing may be requested upon a petition by any group of ten adults. Such petition shall include the name, address and signature of each adult. Written comments concerning the facility’s ability to provide quality long-term care services and petitions for a public hearing, may be addressed to the Department of Public Health, Division of Health Care Facility Licensure and Certification, Suitability Review Office, 99 Chauncy Street, 11th Floor, Boston, MA 02111, or submitted electronically to [email protected], for a period of 14 days following this publication. All written or oral comments submitted to the Department may be posted on the Department’s website and released in response to a request for public records.
